A Servant's Heart Hospice provides hospice care wherever the patient calls home. We bring the services to you!

Our Hospice program is FREE OF CHARGE! • A Servant's Heart Hospice is free to all who have Medicare Part A • Anyone can refer someone to A Servant's Heart Hospice • Our Hospice services are provided in-home, no need to move • Medication related to the Hospice diagnosis are free and delivered • Medical equipment ( beds, wheel chairs, walkers etc) are provided free • Health supplies (briefs, wipes, wound supplies) are provided free • Patients can keep their own doctor or choose our Medical Director • Hospice is NOT for the last days of life, most benefit from months of care • Volunteers help with companionship, errands etc. • Hospice team includes: Dr., RN, CNA, Chaplain, Social Worker, Volunteers • Families benefit from 13 months of bereavement care and support Our H.E.A.R.T outreach pro
