Our role is to help families who are caring for older relatives. We provide a best possible plan of care for our elderly clients in order for them to remain at home as long as safely possible or in the least restrictive environment. We can be a coordinator of existing services (especially at points of transition), an advocate for the appropriate use of services to assure access, a creator of new services either privately or publicly funded, and an integrator of all appropriate public and private funding sources to individualize care for elderly clients and their families.Our Geriatric care managers are members of the National Association of Professional Geriatric Care Managers (NAPGCM) and are committed to adhering to the NAPGCM Pledge of Ethics and Standards of Practice.
